Eli Adler è un rinomato psichiatra infantile di 76 anni, tormentato dall'insonnia e da incubi violenti, in seguito al suicidio della moglie illustratrice Lynn , avvenuto sei mesi prima.
Eli sta pensando di ritirarsi, ma i suoi piani vengono messi in pausa dopo un incontro inquietante davanti alla sua porta. Un giovane, coperto di graffi e simboli incisi sulle dita fino a sanguinare, scappa via prima che Eli possa capire cosa stia succedendo. Con un colpo di scena, Eli viene assegnato al caso del ragazzo, Noah, e scopre che è stato abbandonato da bambino, ha avuto diverse famiglie affidatarie e rischia di essere mandato in un ospedale statale.
Ben presto, sembra che Noah abbia un collegamento inquietante con il passato di Eli, e lo psichiatra diventa ossessionato dal trovare una spiegazione razionale per il comportamento apparentemente soprannaturale del ragazzo, inclusa la sua inspiegabile conoscenza del olandese del XVII secolo e i suoi inquietanti disegni di una misteriosa fattoria.
