Il Reverendo Cotton Marcus arriva nella fattoria di Louis Sweetzer, nella Lousiana, pensando di dover compiere il solito "esorcismo" di routine su una fanatica religiosa affetta da disturbi psicologici. Sweetzer, fervente fondamentalista, ha contattato il carismatico predicatore come sua ultima risorsa, perché è convinto che sua figlia - l'adolescente Nell - sia posseduta da un demone che debba essere esorcizzato prima che una situazione già di per sé terrificante si trasformi in una tragedia inimmaginabile. Cotton è un uomo schiacciato dal peso della sua coscienza, pentito degli anni passati a rubare i soldi alle persone disperate, per questo, assieme al suo team, decide di filmare un documentario-confessione di quello, che sarà il suo ultimo esorcismo. Al loro arrivo, però, la fattoria a conduzione familiare è già immersa nel sangue, e, ben presto, Cotton si rende conto che niente lo avrebbe potuto preparare al male che qui si troverà ad affrontare. Ma è troppo tardi per tornare indietro, e le sue stesse convinzioni vengono scosse profondamente quando si ritrova a dover cercare il modo di salvare Nell, oltre che se stesso, prima che sia troppo tardi.
